What You'll Be Doing:
โ€ข Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines
โ€ข Plan nursing interventions for each resident based on individual needs and monitor the implementation
โ€ข Evaluate the effectiveness of care interventions, identify problems and trends and develop alternative interventions.
โ€ข Conduct family and resident education
โ€ข Administer medications and treatments timely and according to center policy
โ€ข Transcribe physician's orders and chart progress notes in an informative, factual manner that reflects the care administered as well as the resident's response
โ€ข Identify and report changes in condition to supervisor, physician and family
โ€ข Accurately identify skin changes and follow skin management protocols
โ€ข Order and report diagnostic tests timely
โ€ข Follow Pharmacy policy and procedures for ordering and delivering medications.
โ€ข Coordinate resident's rehabilitation program with therapists to ensure rehabilitation needs are met
โ€ข Monitors that CNA documentation is completed daily
โ€ข Ensures CNA assignments are completed on time
โ€ข Perform additional nursing care responsibilities as required and based on State Nurse Practice Act.
โ€ข Requirements:
โ€ข Active state RN license
โ€ข CPR/BLS Certification
โ€ข Experience in Long Term Care or Skilled Nursing preferred
โ€ข Excellent technical, assessment and documentation skills
โ€ข Strong analytical and problem-solving skills
โ€ข Ability to lift 25 or more pounds
โ€ข Ability to stand, bend, and walk for extended time periods
โ€ข Team player, strong communicator, passionate about senior care
