Location: Supporting patients from:Gulf Coast Medical Center,Fort Myers FL

Department: Hospital at Home (Orion Command Center Office/Work location, Orion Drive, Fort Myers, FL)

Work Type: Full Time (72 hours bi-weekly)

Shift: Days, 7:00:00AM to 7:30:00PM

Minimum to Midpoint Pay Rate: Range $35.04 - $47.30 / hour

Lee HealthsHospital at Home programservice brings hospital-level care to eligible patients at home, including daily visits, remote monitoring, and treatments. It helps patients recover comfortably while reducing hospital-related risks. The program reflects Lee Healths commitment to innovative, patient-centered care in Southwest Florida.

This RN Nurse Navigator role is a "hands-off" role. No direct clinical patient care.

Summary

Facilitates a timely, integrated and coordinated approach to the care of acute patients in the home setting (Hospital at Home). Uses a multidisciplinary approach and acts as a liaison between the patient, staff and acute care hospitalist as well as ancillary areas of the hospital system. Identifies learning needs and content for educational programs. Develops, presents, coordinates and evaluates educational activities. Effectively performs in the role of clinician, facilitator, advocate, consultant and/or researcher.

Requirements

Education: Bachelors degree in nursing required, Masters preferred.

Experience: Minimum of 3 years acute care clinical experience. (RN)

Certification: American Heart Association Provider Basic Life Support (BLS) required.

License: Currently licensed RN in the State of Florida.

Other: Strong communication and interpersonal skills with the ability to flex and adapt to evolving environment and role. Computer systems, telephone/video, and customer service skills required.
