Infusion RN- Home Health

As an Infusion Nurse, you will deliver specialized infusion therapies, monitor patient progress, and educate patients and caregivers to ensure safe and effective care. Working under physician-directed plans of care, you'll collaborate closely with the broader clinical team to support optimal outcomes and patient satisfaction.

Core Responsibilities

  • Initiate and manage nursing care plans in alignment with physician orders
  • Administer IV therapies, manage central lines, and perform related clinical procedures
  • Collaborate with multidisciplinary teams including physicians, pharmacists, and therapists to coordinate complex care plans involving cardiac catheterization, dialysis, or specialized therapies within Neurology, Gastroenterology, Rheumatology, and Immunology space primarily.
  • Conduct ongoing patient assessments and respond to clinical changes
  • Educate patients and caregivers to promote independence with care and therapy
  • Document all services in real-time using mobile EMR tools
  • Communicate with physicians, care teams, and caregivers to ensure care continuity
  • Support discharge planning and communicate care summaries
  • Adhere to clinical protocols, HIPAA regulations, and Corewood Care standards

Qualifications

Required:

  • Active RN license in the state of Maryland
  • Current CPR certification
  • 2-3 years of nursing experience (infusion preferred)
  • Valid driver's license, reliable transportation, and auto insurance
  • Basic proficiency in mobile technology and EMR systems

Preferred:

  • Bachelor of Science in Nursing (BSN)
  • 2-3 years of home health or infusion experience

Physical and Safety Requirements

  • Lift up to 50 lbs (occasionally up to 75 lbs)
  • Frequent walking, standing, kneeling, and maneuvering of medical equipment
  • Use of PPE in home settings with variable environmental conditions
  • Potential exposure to bodily fluids and household risks
