Job Title: Home Health Physical Therapist - PartTime
Hourly Rate Range: $60 - $70 per hour
Coverage Area: Merced County, California

Responsibilities:

  • Evaluate and assess patients’ physical conditions and home environments
  • Develop individualized treatment plans to improve mobility, strength, and function
  • Provide hands-on therapy, therapeutic exercise, and patient/family education
  • Collaborate with nurses, aides, and other team members to coordinate care
  • Maintain accurate documentation (EMR experience a plus, OASIS preferred)
  • Supervise PTAs and participate in case conferences and in-service education

Qualifications:

  • Current CA Physical Therapist license
  • Graduate of an accredited PT program
  • CPR certification
  • Valid driver’s license, insured and reliable vehicle
  • Minimum 2 years PT experience (home health preferred but not required)
  • Strong communication and interpersonal skills
  • Knowledge of PDGM Medicare model a plus
