Home Health Weekend Admission Nurse
Pay: $145 per admission, mileage reimbursement and paid drive time
Schedule: Saturday and Sunday required, optional Friday and Monday
Join our compassionate care team as a Home Health Weekend Admission Nurse providing support throughout Salt Lake County. In this role, you'll admit patients, create care plans, and deliver meaningful, patient-centered care in the comfort of their homes.
What You'll Do:

  • Complete admissions and develop individualized Plans of Care
  • Provide skilled nursing visits for home health patients
  • Collaborate with case managers and interdisciplinary teams
  • Educate patients and families while supporting comfort and dignity
What We're Looking For:
  • Active Utah RN license
  • 2+ years nursing experience (home health or hospice preferred)
  • Strong assessment, communication, and time management skills
  • Reliable transportation and CPR certification
