Traveler Requirements

Candidates with an AZ home address on their driver's license will not be considered. Please include a copy in submission Banner has a specific cover page! PLEASE vet all locations to the travelers and add all experienced departments candidates are willing to ACCEPT/work in. This helps us get candidates presented quicker as we all know these orders open/close quick! This form needs to be 100% accurate and have all request noted in the highlights section.

Start Date should be on or very quickly after the Order Start Date listed.

All RTO should be listed on the cover page, must be less than 7 days.

RTO will not be approved in the first week of assignment or holidays.

All facilities the traveler is open to working in should be listed on the submission.

If interested in "any" location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)

All shifts the traveler is open to working on should be listed on the submission.

All departments the traveler is open to working must be checked on the cover page.

All candidates submitted should be preclosed and ready to accept.

Please let us know if the traveler withdraws or is no longer available.
