Night RN Support

Partners In Care (PIC) is an independent 501(c)(3) nonprofit and the region's largest, longest-standing provider of home health, palliative care, and hospice services in Central Oregon. We serve Deschutes, Crook, and Jefferson counties with a dedicated team of more than 200 employees and 200 volunteers, delivering compassionate, patient-centered care across a 10,000-square-mile region. Our Hospice House in Bend is a specialty inpatient, acute care hospice facility—an oasis of comfort and peace for patients and their families. Position Details

Full-Time, hourly, non-exempt. Three 15-hour shifts per week.

Night Shift Hours: 5 PM-8 AM. Work outside of these days/hours may be required.

$42.00-60.53 per hour, based on experience. Eligible shifts receive additional differential and standby pay.

Working all throughout Central Oregon in patients' homes and facilities.

Summary/Objective

The Night RN Support provides comprehensive hospice care across multiple settings during the night hours. This dynamic role primarily focuses on field-based, on-call responsibilities, delivering timely and compassionate care to patients and families in their homes. When not actively responding to field calls, the RN will provide meal and rest break coverage, as well as light patient care, at our inpatient Hospice House. Additional duties may include assisting with chart audits to support continuous quality improvement across the organization. These secondary duties support continuity of care and uphold clinical excellence.

Essential Functions Primary Responsibilities-After Hours Field RN Support:
  • Develop an individualized care plan that establishes goals based on nursing diagnoses and incorporates therapeutic, preventive, and rehabilitative nursing actions. Include the patient and the family in the planning process.
  • Provide nursing procedures and treatments, and/or end-of-life comfort procedures, per the plan of care. Administer medications and treatments as prescribed by the physician.
  • Complete and update the ongoing assessments and care plan as the patient's status changes.
  • Ensure that arrangements for equipment, other necessary items, and services are in place.
  • Provide health care instructions and education to the patient and or family as appropriate per the assessment plan and the patient's caregivers' ability to learn. Adapt teaching styles to meet patients' and families' needs.
  • Coordinate patient care with the patient's case manager. Provide verbal or written updates as necessary.
  • Communicates with the physician regarding the patient's needs, reports, and changes in the patient's condition; obtains/receives the physician's orders as required.
  • Communicate with all members of the healthcare team to promote coordinated, efficient care and document coordination of care with all involved with patient care.
  • Ongoing clinical documentation is completed, accurate, documented, and submitted within 24 hours of the patient visit.
  • Communicate with facility staff, providing written updates and orders as requested.
  • Local/regional travel by personal vehicle during the working day, centralized from the Bend office and extending throughout Central Oregon.
Secondary Responsibilities-Hospice House Support and Chart Auditing:
  • Relief coverage for Hospice House staff during scheduled meal and rest breaks when available, ensuring continuity of patient care and team support. This backup responsibility helps maintain a smooth workflow and supports staff well-being during overnight shifts
  • Under the direction and delegation of the shift Lead at Hospice House, apply nursing principles and techniques in the treatment of patients. Provide nursing care, emotional support, and education to the patient/family/caregiver. Maintain strict confidentiality of patient, family, and caregiver information.
  • Follow the guidelines for universal precautions when providing patient care and demonstrate proper techniques in application of infection control standards.
  • Maintain records of services performed and of apparent condition of patient, using documentation guidelines and standards set forth by the facility.
  • Communicate with physicians and others as needed to coordinate and ensure high quality care. Communicate on a regular basis with all staff / volunteers regarding patient status.
  • Demonstrate a working knowledge of the dying process and recognizes the importance of and maintains professional boundaries.
  • Provide for a safe environment for patients, guests, visitors, and employees and adhere to all policies, procedures, and regulations regarding patient care, conduct, safety, fire, security, and risk management.
  • Supports clinical quality initiatives through chart auditing when not actively engaged in field-based patient care. This includes reviewing documentation for accuracy, completeness, and compliance with hospice standards to help ensure regulatory readiness and promote continuous quality improvement across the organization.
Work Environment:
  • May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances.
  • May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations.
  • Handles emergency or crisis situations.
  • OSHA exposure category 1: Position includes tasks that involve exposure to blood, body fluids, tissues.
  • Required Personal Protective Equipment as conditions warrant: Gloves, gown, mask, mask with shield, goggles, shoe covers, bouffant cap.
Required Skills & Qualifications:
  • Graduate of an accredited school of nursing with an unencumbered licensure to practice professional nursing in the State of Oregon.
  • One to two years of recent medical/surgical care experience in an institutional setting.
  • Ideal candidates will possess 1-2 years of home health or hospice experience.
  • Valid CPR Certification.
  • Current automobile insurance, a valid Oregon driver's license, and reliable transportation are required for those working in the field.
  • Ability to pass a reference check, background test, drug screen, and TB test.
  • Strong communication and interpersonal skills.
  • Excellent assessment, problem-solving skills, and nursing skills per competency check.
  • Strong clinical judgment and patient triaging skills are vital; efficient time management with minimal oversight; communication skills (with other care team members, including physicians); positive phone interaction/customer service; and computer skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Compassion, empathy, and dedication to patient care.
