The role of Licensed Clinical Social Worker at Thrive is uniquely designed for a remote work setting, allowing you to provide services from the comfort of your home while still making a significant impact in the community. Our Hospice Center functions as a vital support hub, where you will work with a diverse team of healthcare professionals dedicated to improving patient outcomes. You will utilize our state-of-the-art electronic health record (EHR) system to document assessments, treatment plans, and progress notes securely.

• Conduct thorough biopsychosocial assessments to evaluate the mental health, substance use, and medical needs of patients in a primary care setting, ensuring that assessments are age and culturally appropriate.
• Provide a range of behavioral health services, including individual and group counseling, crisis intervention, intake assessments, and ongoing treatment planning.
• Utilize effective counseling techniques to develop and facilitate supportive psychosocial interventions, promoting emotional well-being and resilience in clients.
• Act as a liaison between clients and local resources, connecting them with necessary support services, including community agencies and medication assistance programs.
• Collaborate with internal Quality Improvement teams to enhance data collection processes, contributing to the continuous improvement of service delivery and patient care.

The clients you will serve include a diverse demographic, ranging from individuals facing mental health challenges to families in need of support during times of crisis. You will focus on delivering compassionate care tailored to each client’s unique cultural and personal background, ensuring their treatment plans align with their specific needs and circumstances.

• A valid state license as a Licensed Clinical Social Worker (LCSW) is required.
• A Master’s degree in Social Work is mandatory, coupled with a minimum of three (3) years of post-licensure experience.
• Proficient knowledge of various counseling techniques, crisis intervention modalities, and the ability to exercise independent judgment regarding client treatment is essential.
• Experience with group facilitation and planning processes, underscoring your skill in developing interactive sessions that foster community and support.

• Familiarity with electronic health record (EHR) systems, particularly in documenting mental health assessments and treatment plans.
• A history of working within a hospice or palliative care setting, understanding the unique dynamics of such environments.
