Job description


Job Title: Case Counselor

Reports to: Niagara County Youth Services Program coordinator

Job Category: Professional Non-Exempt

Department: Niagara County Youth Services

Salary (range): $45,000 - $47,400

Position Status: Full-time


Position Purpose: A Case Counselor's primary role is to provide family focused services, linkages and referrals to community resources and coordinates care plans for involved youth and their families.


PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Engages the Youth Services Team in addressing needs that impact each youth's success.
  • Provides outreach, engagement, and enrollment activities concerning referred for services.
  • Provides additional activities that include mailings, electronic communications and telephone calls with consumers, their families/significant others, and various community service providers.
  • Works effectively with all colleagues to identify provide necessary services to all consumers.
  • Function as an ambassador for BFNC at community events and gatherings
  • Attend family and criminal court appointments as scheduled
  • Must have use of an automobile and valid driver's license
  • Assist in the planning and organization of company events
  • Overall responsibility for the individual and group counseling
  • Implement case management of youth
  • Coordinates family related services
  • Attends all necessary court appointments
  • Conduct in-take interview and assessment
  • Develop plan of care for youth and their family
  • Maintain accurate records to ensure timely and accurate reporting


KNOWLEDGE AND SKILL REQUIREMENTS

Qualifications: Associate Degree in Human Services, Criminal Justice, Humanities or High School Diploma/GED with 2 years of experience working with families who have children who are at risk of school failure or becoming involved with the criminal justice system.


Must demonstrate the abilities and experience to:

  • Maintains confidentiality in all interactions with stakeholders and service providers
  • Demonstrate the ability to maintain an even temperament in crisis or emergency situations and consistently use superior interpersonal skills in collaborating with other providers and consumer-centered community support systems
  • Value cultural difference, collaborative efforts, and person-centered services
  • Ability to work independently, using time management and organizational skills
  • Demonstrate excellent verbal, written, and documentation skills
  • Strong judgment, problem solving, and time management skills are essential
  • Must exhibit a passion for BFNC's vision, mission and values and act as a role model and ambassador for agency values and responsible practices
  • Proficient in Microsoft Office and all other required technology expertise with the proven ability to enter data into BFNC systems

Ergonomics:

Sitting, standing, stooping, bending, standard lifting up to 25 pounds, carrying, repetitive movements. Ability to operate and repair machines; prolonged periods of standing, walking, bending, and stretching. Must be able to access and navigate the site and go up and down stairs. All the ergonomic requirements must be performed without significant risk of injury to oneself or others or otherwise demonstrated or explain how one can perform essential functions of the position listed above with or without accommodations.
