Job description

Provide evidence-based primary care within a team-based medical home model, partnering with patients and the care team to develop biopsychosocial care plans, manage acute and chronic conditions, and support population health and quality improvement.

Key responsibilities

  • Deliver primary care to ~10–15 patients/day: prevention, acute and chronic disease management, and case management.
  • Develop individualized care plans in collaboration with patients and team members, including an onsite Whole Health Coach.
  • Perform comprehensive health histories, physical exams, diagnostic testing, and prescribe medications; onsite lab and pharmacy available.
  • Drive participation in whole health evaluations, health coaching, and other health-promotion activities based on patient-population needs.
  • Provide clinical leadership: participate in team meetings, communicate proactively, prioritize initiatives, support process improvement and change management.
  • Rotate with regional providers for after-hours coverage.

Compensation & benefits

  • Base salary: $150,000.
  • Premium medical, dental, vision coverage; HSA pre-tax option.
  • Medical education: 2 weeks plus $2,500 stipend.

Requirements

  • NP or PA board certification in primary care, family medicine, or internal medicine.
  • Current, unrestricted ARNP or PA license and DEA number.
  • 3–5 years primary care experience.
  • ≥2 years case management experience with emphasis on prevention and population health management.
  • Experience with EMR systems (NextGen preferred).
  • Experience in coding and documentation, including Medicare Advantage and HCC.
