As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient's care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Schedule: Monday - Friday 8:00am-5:00pm, with occasional on-call
Location: Whitley, Laurel, Clay, and Knox Counties. Occasional coverage to Bell and Harlan counties.
Position Summary:
The Hospice Flex RN is a diverse role providing support to Hospice RN Case Managers to ensure patient needs are met and high-quality care is maintained in various home care settings, such as private residences, skilled nursing facilities, personal care homes, or wherever the patient calls home. The Hospice Flex RN is dynamic depending on a regional office's most urgent needs and can involve providing care to specific caseloads, completing visits for multiple case managers, assisting with new admissions and managing referrals, and participating in on-call rotations to ensure continuous patient care. A Registered Nurse in this position must possess strong assessment skills and a thorough understanding of disease processes, as well as the ability to manage symptoms proactively. Excellent communication skills are essential for interacting with patients, families, and interdisciplinary team members. The Hospice Flex RN must be able to work autonomously, handle various patient care scenarios with professionalism, and provide empathetic and compassionate care in emotionally challenging situations. This position offers ongoing opportunities to make a significant and rewarding difference in patients' lives during a challenging time.
Position Qualifications:
• Graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or through a participating Compact State Board of Nursing.
• Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered.
• Demonstrates strong clinical nursing skills and the ability to maintain professionalism during emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
• Adheres to accepted professional, ethical, and legal standards of nursing practice.
• Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
• Maintains current CPR certification.
• Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
