Your Key Responsibilities as a Physical Therapist
- Perform initial/ongoing assessments (including OASIS) to identify needs, functional levels, and home health eligibility
- Create and update personalized Plans of Care with physicians and care team
- Deliver therapeutic treatments and assess equipment needs
- Track outcomes and coordinate effective discharge planning
- Apply infection control and standard precautions
- Manage assignments within your education/competence
- Supervise PTAs and HHAs appropriately
- Document accurately and timely
- Fulfill CE requirements and join in-services
- Communicate effectively with patients, families, and staff
- Champion agency philosophy, policies, and professional growth
- Use technology confidently for all job functions
Qualifications We're Seeking
- Education: BS in Physical Therapy (or higher) from an APTA-approved program
- Licensure: Active Texas PT License + valid Texas Driverโ€™s License
- Skills: Solid therapy practice standards, great interpersonal skills, current BLS CPR (healthcare provider), DME knowledge
- Transportation: Reliable vehicle with current auto liability insurance
- Experience: Home health preferred; EMR HomeCare HomeBase a plus
