Position Summary

The primary responsibility of the Nurse is to support the overall health and well-being of individuals served by AMA Support Services through coordination, education, and oversight of health-related records and needs in a non-clinical setting.

Reporting to the Program Director, this role focuses on clinical records and assessments to guide and recommend appropriate health supports, follow-up care, and training for caregivers who support the general health needs of people in service. 

The Nurse is a non-clinical position which does not provide ongoing direct clinical treatment. Instead, the nurse uses their medical expertise and knowledge to serve as a liaison between individuals, healthcare providers, and staff to ensure continuity of care and overall well-being.

Job Duties Include

  • Represent the Mission, Vision, and Values of AMA Support Services while promoting dignity, respect, and person-centered support
  • Support Program Management in providing healthcare services, track follow-ups, and offering recommendations and advice in ways that help to promote continuity of care 
  • Act as liaison with physicians, specialists, pharmacies, and other medical providers 
  • Conduct nursing assessments as needed, administer injections, and oversee monthly vital sign clinics
  • Complete HRST assessments to ensure accuracy, appropriateness, and identification and implementation of recommended supports
  • Monitor clinical heath records in the agency’s Electronic Health Record system, such as Medication Administration Records, intake and elimination records, and all others, and recommend or advise as appropriate
  • Collaborate with the Training Manager to develop and deliver staff training, including, but not limited to, all topics required by regulation, individual and trend data from HRST reports and other clinical records, and other health and wellness related topics deemed appropriate
  • Support wellness initiatives and committee involvement, provide reports and information from clinical record sources, make recommendations for individual and group health and wellness initiatives and goals
  • Serve as a health resource to staff and individuals by recommending and advising on health-related matters, identify health concerns, and maintain accurate documentation in compliance with agency and regulatory requirements

Required Education and Experience

  • Valid and unrestricted Registered Nurse (RN) license in the Commonwealth of Pennsylvania
  • Graduation from an accredited nursing program and successful completion of the NCLEX-RN exam
  • Experience in home health, community-based services, Nursing home, or working with individuals with IDD preferred
  • Familiarity with HRST, OASIS, or MDS documentation preferred

  • Critical thinking, problem-solving, and decision-making skills to effectively respond to individual needs
  • Effective communication and interpersonal skills for collaborating with individuals, families, healthcare providers, and program teams
  • Ability to demonstrate empathy, professionalism, and compassion while managing a variety of situations
  • Strong organizational skills, attention to detail, and proficiency in EMR systems, with the ability to work independently and as part of a team while managing multiple priorities

Working Conditions

  • Hours and weekly schedule are dictated by current tasks and program needs. 
  • Work will take place in a variety of settings, including an office, in the community and in the homes of the people in service. Must be able to travel to and work in these settings without limitation 
  • Exposure to various medical conditions and communicable diseases may occur 
  • Schedule flexibility based on program and appointment needs
  • Minimal physical demands. Occasional lifting, stretching or assisting may be required
