RHTP NavigatorPay Range: $45,000-$50,000Location: Bollinger County, MOPosition: Full-TimeSchedule: Monday-Friday, 8:00 a.m.-4:30 p.m. (times may vary; occasional evening or weekend outreach may be required)Travel: Frequent local travel throughout Bollinger County; travel is expected most daysMinimum Required QualificationsHigh School Diploma / GED / HSE Requiredand relevant experience in community health work, care coordination, case management, patient navigation, Medical care management, behavioral health support, public health outreach, social services, or a related field. Experience working directly with rural communities and/or individuals facing barriers to careExperience completing client, patient, or resident intake/needs screenings and following referrals through to completionAbility to manage multiple active cases while maintaining organized and consistent follow-upMust hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents), and proof of continuous auto insurance. Must also meet CCC's Motor Vehicle Record standards policy and have reliable transportation.The RHTP Navigator RoleCommunity Counseling Center (CCC) is seeking an RHTP Navigator to serve as a frontline, community-based member of Local Hub 25 in Bollinger County.Through Missouri's Transformation of Rural Community Health Care (ToRCH Care) model, the Navigator helps residents connect to and successfully complete needed healthcare, behavioral health, and community services.This is a non-clinical, field-based position that works throughout the community, including homes, clinics, hospitals, schools, pharmacies, EMS/community paramedicine sites, local public health agencies, and other partner organizations.You'll build relationships with residents, identify barriers to care, connect people with resources, and follow up to make sure needed services are actually received.What You'll DoConduct screenings and intake to identify healthcare, behavioral health, and social needsConnect residents with appropriate healthcare and community servicesManage referrals and warm handoffs between residents and Hub partnersFollow up with residents and providers to confirm services were received and needs were addressedMaintain an assigned caseload and provide consistent follow-up until services are completed, needs are resolved, or care is appropriately transitionedHelp residents overcome barriers such as transportation, medication access, insurance or benefits issues, food insecurity, and limited digital access and assist residents in accessing services such as primary care, behavioral health, women's and prenatal care, children's preventive services, chronic disease management, pharmacy services, telehealth, home visiting, and non-emergency medical transportation.Engage Medicaid members and other residents with complex needs through phone calls, community outreach, field visits, and partner-site follow-upProvide health education, care navigation, and reinforcement of established care plansDocument screenings, outreach, referrals, follow-up, and outcomes using the Community Information Exchange (CIE) and other approved systemsRe-engage residents who are difficult to reach or who have not completed recommended servicesParticipate in Hub huddles, case reviews, and partner meetingsBuild strong working relationships with hospitals, healthcare providers, behavioral health providers, pharmacies, schools, public health agencies, EMS/community paramedicine programs, and community organizationsMaintain a visible presence throughout the community through outreach, partner engagement, and local eventsIdentify recurring barriers or service gaps and share insights that can improve local programs and referral processesEscalate urgent medical, behavioral health, or safety concerns to licensed staff or supervisors according to established protocolsOther duties as assignedCommunity Counseling Center is an equal opportunity employer, and all qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability status, protected veteran status or any other characteristic protected by law.CCC will consider requests for reasonable accommodations for qualified individuals with disabilities in accordance with the Americans with Disabilities Act (ADA).
