Job Description

The Division of Gastroenterology at the Icahn School of Medicine at Mount Sinai is looking for a Patient Navigator. The Patient Navigator oversees a caseload of patients and acts as the primary liaison between the patient and the hospital system; this involves scheduling and helping to complete their colonoscopy. The Patient Navigator can also work as a lead Patient Navigator, which is a liaison between less experienced Patient Navigators and management. In this role, you are responsible for implementing the comprehensive care plan to each patient as determined upon enrollment in the program. The Patient Navigator ensures that the patient adheres to the established plan of care.

As a Program, our work is focused on helping to reduce health disparities and ensure anyone and everyone can have access to the care they need. As an integral part of our colorectal cancer screening program, we are seeking a Patient Navigator to join our team to assist patients in completing their colorectal cancer screening and also serve as a mentor less experienced Patient Navigators.

Qualifications

  • Bachelor's degree preferred or High School Degree and relevant work experience

  • 2 years minimum experience, 3-5 highly preferred

  • Flexible work schedule - will need to work at least 2 evenings and potentially some Saturdays

  • Experience as a Patient Navigator or similar position

  • Experience with Excel and other computer software

  • Excellent communication and telephone skills

  • Bilingual in Spanish preferred

  • Experience with EMR systems, preferably EPIC

Responsibilities

  • Schedule and navigate direct access colonoscopies that are referred to The CHOICE Program

  • Contact patients to schedule colonoscopy procedures in accordance with our policies and procedures

  • Works to establish a relationship with each patient and opens up a line of communication.

  • Confirms with each patient that they are willing and committed to completing a colonoscopy.

  • Addresses and answers any questions/concerns/issues from patients and less experienced Patient Navigators

  • Schedule the procedure and provide patients with instructions on preparation for procedure, need for transportation and escort

  • Assist with verifying insurance and insurance related matters

  • Provide ongoing navigation - this may include but is not limited to reminder phone calls, text messaging, preparation calls and reviewing the hospital system's policies and procedures. This may entail numerous phone calls

  • Track all encounters in the CHOICE tracking database Compass Rose

  • Assist with scheduling and navigating GI Fellows Clinic patients, in person and remotely

  • May perform duty as liaison between Patient Navigator staff and Management in addressing patient concerns

  • May participate and offer input into the Performance Appraisal process with Management.

  • Trains and mentors less experienced Patient Navigators and discusses issues of concern with management

  • May be asked to participate in data projects to help advance the goals of the program

  • As a Navigator, may be asked to cover other Navigators as well as the manager in case of the manager's absence

  • Performs other tasks as required.

Compensation Statement

Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $22.5024 - $25.021 Hourly. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
