Job description

Job Summary
Registered Nurse (RN) Care Manager (RNCM)
Overview: Aptive Resources, in partnership with Valor Healthcare, is hiring a Registered Nurse Care Manager to provide care coordination and chronic disease management within the PACT model at the proposed Yreka, CA CBOC, pending VA contract award.
Location: On-Site | Yreka VA Community-Based Outpatient Clinic (CBOC), 807 S. Main St., Yreka, CA 96097
Schedule: Monday-Friday, 7:30am-5:30pm (Clinic Operating Hours); 8:00am-4:30pm Patient Schedule Hours
Primary Responsibilities
  • Provide professional nursing services consistent with licensure, certification, and assigned functional statement
  • Collaborate with PACT staff to develop and manage care management plans and care coordination for the assigned panel
  • Participate in secure messaging, virtual care, shared medical appointments, and face-to-face visits as part of care management
  • Identify needs for discipline-specific referrals and enter consultation requests as needed
  • Enter orders in the VA EHR for tests per approved standardized RN protocols or PCP orders
  • Use evidence-based guidelines and nursing protocols to promote patient engagement, self-care, and wellness
  • Maintain >90% completion rate for preventive screenings; document interactions in CPRS within 72 hours

Minimum Qualifications
  • Graduate of an ACEN- or CCNE-accredited school of professional nursing
  • Current, full, active, unrestricted RN registration
  • Current BLS certification
  • Experience in care coordination and chronic disease management
  • Must hold a current, full, active, and unrestricted license/certification (as applicable to the position) in California, or otherwise meet VA Qualification Standards for the position.
  • Must successfully complete VA credentialing/privileging review and a background investigation (cleared through OPM via the VA Security and Investigations Center) prior to starting work; PIV credentialing required.
  • Must complete required VA mandatory training and maintain current licensure/certification documentation annually.
