Job Opportunity

Responsible for assisting the patient with activities of daily living and personal care. This position, in collaboration with patient, family, and health care team, will provide direct and indirect nursing care of the patient utilizing the nursing process under the supervision of a Registered Nurse according to the following work systems/functions.

Minimum Qualifications:

  • High school diploma or equivalent.
  • Certification issued by state where services will be performed:
  • MD: Certified Nursing Assistant through the Maryland Department of Health
  • OH: Nurse's Aide through the Ohio Department of Health
  • PA: Nurse's Aide through the Pennsylvania Department of Health
  • WV: Nurse's Aide Certification through the West Virginia Department of Health and Human Resources
  • Valid driver's license.
  • CPR Certification required within 30 days of hire date.

Core Duties and Responsibilities:

  • Provides personal and hygienic care every visit as per care plan and basic care standard in accordance with age-specific needs and policy and procedure.
  • Maintains a clean, safe, and comfortable environment per the basic care standard within the limitations of the home setting.
  • Ensures privacy and demonstrates concern for dignity of patient and respects patient's property in performing all patient care.
  • Completes assignments in a timely manner as per standard.
  • Follows plan of care assigned by RN.
  • Relays critical information to the appropriate staff regarding patient condition.
  • Confers with the primary nurse on the original plan of care and any subsequent updates.
  • Documents patient status and care of services furnished in appropriate areas as defined by the documentation policy and procedure.
  • Reports observed changes in patient condition to R.N.'s, including recognition and reporting of changes in level of comfort.
  • Reports abnormalities in vital signs to R.N.
  • Utilizes supplies effectively and to ensure cost containment.
  • Perform the following treatments/procedures according to the specific standards including but not limited to:
    • Measuring vital signs
    • Lifting and moving patients
    • Positioning the patient in bed
    • Transferring patient from bed to chair
    • Ambulating patient
    • ROM
    • Bed-making
    • Personal Hygiene
      • Oral care โ€“ brushing teeth, denture care
      • Bathing โ€“ bed bath, tub bath, shower
      • Back massage
      • Perineal Care
      • Hair Care โ€“ brushing and combing, shampooing in tub or shower, at sink, in bed
      • Shaving
      • Nail Care
      • Changing clothes
    • Assisting with elimination
      • Bed pan
      • Urinal
      • BSC
      • Ostomy care
    • Patients' weights
    • Case care
    • Identifies the stages of death and dying.

    Physical Requirements:

    • Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, and grasping are necessary body movements utilized in performing duties through the work shift.
    • Must be able to lift, move, and position patients of all weights, with varying amounts of assistance in a fair manner on a regular and recurring basis.
    • Hearing within normal range is required (e.g., apical pulse, monitors, etc.).
    • Visual acuity must be within normal range.
    • Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work.
    • Must be able to operate motor vehicle with a safe practice.

    Working Environment:

    • Exposure to bodily fluids, human tissues, contagious diseases, sharps, and explosive gases.
    • Exposure to cleaning agents and disinfectants.
    • Exposure to toxic gases, fumes, and odors.
    • Exposure to high stress and constant interruptions.
    • Exposure to electrical current.
    • Exposure to radiation from x-rays.
    • Exposure to wind, rain, snow, sleet, mud, high and low temperatures, animals and insects, and hazardous roadways.
    • The majority of work is performed in the patient's home and may require extensive travel.
    • Subject to unpredictable situations. May be exposed to adverse or unpleasant home environments.

    Skills and Abilities:

    • Must have the ability to perform concentrated and complex mental activity with frequent involvement in complex and highly-technical situations.
    • Must have the ability to work successfully under highly-stressful conditions, and must be capable of adapting to varying workloads and work assignments on a constant basis.
    • Must have the ability to make sound, independent judgments and also be able to collaborate with other multi-disciplinary team members in an appropriate fashion.
    • Must have the ability to comprehend and perform oral and written instructions and procedures.
    • Must have effective reading and comprehension skills.
    • Must have strong communications skills, written and verbal.
    • Must have basic math skills.
    • Must be able to work weekends, holidays, and all shifts.
    • Must be able to operate telephone, fax machine, beepers, and computer appropriately in order to maintain flow of information.
    • Ability to read and write legibly.
    • Demonstrates knowledge of and compliance with safety policies and procedure with side rails, fall risks, TB precautions, and hand washing.
    • Demonstrates knowledge of specialized needs of the terminally ill patient per philosophy, policy, and procedure.
    • Demonstrates knowledge of compliance of Federal, State, and Local regulations/laws.
