Job description

Case Manager - Brighton Center For Recovery 

Be a vital advocate at the center of patient care.

In this role, you will lead and coordinate the interdisciplinary plan of care, guiding patients and families seamlessly from admission through discharge. You'll serve as a trusted partner to patients, providers, and leadership-ensuring care is collaborative, compassionate, and aligned with individual needs and values.

  • Monday-Friday work schedule (8am-530pm)
  • Occasional holiday coverage may be required
  • Pay Range: 23.26/hr / 23.76/hr / 25.88/hr

Responsibilities: 

  • Design and coordinate comprehensive care plans across the continuum, integrating patient and family goals, preferences, and clinical needs.
  • Monitor and evaluate patient progress through ongoing assessments, care evaluations, and medical record review.
  • Advocate for patients by identifying needed resources, addressing barriers to care, and promoting timely, appropriate services.
  • Collaborate closely with physicians, clinicians, and leadership, maintaining open communication to ensure effective execution of the care plan.
  • Serve as a clinical documentation and utilization resource, supporting accurate medical records, appropriate care levels, diagnosisdriven services, and wellcoordinated postdischarge plans.

Qualifications: 

Master's degree in Social Work (MSW) or Registered Nurse (RN) license required
