Geriatric Chronic Condition Management, 3-days per week (Monday, Tuesday, & Friday). Value-based care model, delivering primary care to patients in  Kent, Federal Way, Auburn and Renton areas.

We seek a Value Based Care Nurse Practitioner to support a growing team in South Seattle. You will spend 3045 min building relationships with your own patient panel addressing all chronic conditions, giving you time to achieve HEDIS and quality-focused patient outcomes.

Responsibilities:

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patients needs. Manage chronic conditions of assigned patient panel through 30-45 min patient visits. Achieve quality outcomes meeting HEDIS/STAR measures.

Qualifications:

• Licensed as a NP in the State
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid drivers license and good driving record
