Position Summary

The Admissions Director is responsible for overseeing all aspects of the admissions process while driving census growth through strategic relationship building with hospitals, physicians, case managers, social workers, discharge planners, and referral partners.

This position serves as the primary liaison for prospective residents and their families, ensuring a seamless, compassionate, and professional admission experience from initial inquiry through move-in.

The ideal candidate will have experience in Skilled Nursing (SNF), Long-Term Care (LTC), Rehabilitation, Healthcare Marketing, Business Development, Hospital Liaison services, or Case Management.

Responsibilities
  • Lead the admissions process from initial referral through resident admission.

  • Build and maintain strong referral relationships with hospitals, physicians, discharge planners, case managers, social workers, assisted living communities, hospice providers, and other referral sources.

  • Develop and execute strategies to increase referrals and achieve census and occupancy goals.

  • Conduct pre-admission screenings to determine clinical and financial eligibility.

  • Verify Medicare, Medicaid, Managed Care, commercial insurance, and private pay coverage.

  • Conduct professional facility tours for prospective residents and families.

  • Educate residents and families about available services, rehabilitation programs, insurance coverage, and the admissions process.

  • Coordinate admissions with Nursing, Therapy, Social Services, and Administration to ensure smooth resident transitions.

  • Maintain accurate admission documentation within the Electronic Medical Record (EMR) system.

  • Ensure compliance with CMS, Medicare, Medicaid, state, federal, and facility regulations.

  • Track referral activity, admissions metrics, occupancy trends, and census goals.

  • Represent the facility at networking events, hospital meetings, and community outreach activities.

  • Deliver exceptional customer service while creating a positive experience for every resident and family.

QualificationsRequired:
  • Minimum of two (2) years of experience in Healthcare Admissions, Healthcare Marketing, Business Development, Case Management, or a related healthcare role.

  • Experience in a Skilled Nursing Facility (SNF), Long-Term Care (LTC), Rehabilitation Center, Hospital, or Post-Acute Care setting.

  • Strong understanding of Medicare, Medicaid, Managed Care, and Skilled Nursing admissions.

  • Excellent communication, relationship-building, and customer service skills.

  • Strong organizational, multitasking, and problem-solving abilities.

  • Proficiency with Electronic Medical Records (EMR) and Microsoft Office.

Preferred:
  • Bachelor's Degree in Healthcare Administration, Business Administration, Marketing, Social Work, or a related field.

  • Existing relationships with hospitals, physicians, and community referral sources.

  • Proven success in census development and referral management.
