We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team at the Margaret E. Moul Home where we specialize in resident care dedicated to supporting individuals with neuromuscular conditions. This role offers an opportunity to make a meaningful difference in the lives of residents by providing high-quality, personalized care in a supportive and collaborative environment.

Key Responsibilities:

  • Administer medications and treatments as prescribed by physicians, ensuring accurate documentation and timely delivery
  • Monitor residents’ health status, including vital signs, symptoms, and overall well-being
  • Assist residents with activities of daily living such as bathing, dressing, mobility, and personal hygiene
  • Collaborate with interdisciplinary team members to develop and implement individualized care plans
  • Educate residents and their families about health management and condition-specific needs
  • Maintain accurate and detailed medical records in compliance with regulatory standards
  • Respond promptly to medical emergencies and escalate concerns to appropriate healthcare professionals
  • Ensure a safe, clean, and comfortable environment for residents at all times

Requirements:

Skills and Qualifications:

  • Valid and current Licensed Practical Nurse (LPN) license in PA
  • Prior experience in a long-term care, residential, or neuromuscular care setting preferred
  • Knowledge of neuromuscular conditions and related care requirements preferred
  • Strong interpersonal and communication skills, with a compassionate approach to resident care
  • Ability to work effectively as part of a multidisciplinary team
  • Excellent organizational skills and attention to detail
  • Ability to handle emergency situations calmly and efficiently
  • Basic computer skills and familiarity with electronic health records (EHR) systems
