Key Responsibilities
  • Conduct thorough assessments, ensuring that the unique needs of each patient are accurately identified and addressed through customized care plans.
  • Engage in proactive communication with supervisors, peers, and family members to keep all stakeholders informed on patient progress, strengthening support systems.
  • Draft and maintain detailed scheduling of appointments, home visits, and observation sessions to monitor patient development and program implementation.
  • Coordinate and perform home visits as necessary, gathering vital information through direct observation, chart reviews, and staff interviews.
  • Ensure that all components of the mental health program plan are effectively executed, contributing to improved patient outcomes.
  • Participate actively in interdisciplinary team meetings, bringing insights that promote cohesive approaches to patient care.
  • Create and complete monthly and quarterly progress reports that outline patient developments and recommend potential adjustments to treatment plans.
  • Organize training and educational resources for staff to maintain high competency levels in care delivery and documentation processes.

  • Maintain organized records to guarantee that care information is accessible and transparent for authorized parties.

Patient Population / Client Focus

Qualifications
