Schedule: Monday-Friday | 8:30 AM-5:00 PM
Location: Desert West, Desert East, Desert Central, Desert North, Desert South | Travel within your district required
OT Available
Pay: $37.12, $0.67/ mile travel reimbursement
What You'll Do:
  • Conduct in-person nursing assessments and determine skilled nursing needs
  • Coordinate services with members, families, providers, and community agencies
  • Complete documentation, referrals, and care coordination
  • Participate in multidisciplinary meetings and support special projects
What You Need:
  • Active, unrestricted Arizona RN license
  • 2+ years RN experience in home health, hospice, LTC, or case management
  • Strong communication, assessment, and documentation skills
  • Microsoft Office/Google Workspace experience
  • Level 1 Fingerprint Clearance Card
  • CPR/BLS Card
  • TB Skin Test
  • Agency Skills Assessment
