• Conduct thorough evaluations, set individualized goals, and develop tailored treatment plans for patients referred by physicians, ensuring effective recovery strategies.
• Provide hands-on treatment in a variety of modalities that may include manual therapy, therapeutic exercise, and functional training to address specific patient needs.
• Maintain accurate and timely documentation of patient progress and treatment results, utilizing various healthcare management systems.
• Engage in regular communication with physicians and other healthcare professionals to coordinate care and optimize patient outcomes.
• Lead educational sessions for patients and their caregivers, equipping them with the knowledge and skills necessary for successful home care and self-management.
• Monitor and encourage adherence to treatment protocols while adjusting strategies as needed, ensuring all patient interactions are constructive and motivating.
• Participate in team meetings and case discussions to share insights and best practices that contribute to overall clinic performance and patient satisfaction.

As an Orthopedic Physical Therapist at Montefiore Mobility Rehab Clinic, you will work primarily with a diverse patient population experiencing orthopedic conditions. This includes individuals recovering from surgeries, sports injuries, and chronic pain issues. Your expertise will facilitate targeted interventions to help these patients regain mobility, strength, and confidence.

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid Illinois State License in Physical Therapy.
• A minimum of two years of clinical experience in outpatient orthopedic physical therapy.
• Basic Life Support (BLS) Certification from the American Heart Association.
• Exceptional communication skills—both written and verbal—enabling effective interactions with patients and professional peers.
• Strong organizational and computer skills for managing patient records and documentation.
• A demonstrated ability to build rapport and maintain productive relationships with patients and their families.

• Experience treating a wide variety of orthopedic conditions is preferred, along with familiarity with evidence-based practice methodologies.
• Proficient in utilizing therapeutic technology and equipment to enhance patient treatment outcomes.
• Experience with community outreach or involvement in professional networks to promote health and wellness initiatives.
