Outpatient RN Care Manager - Napa, CA
**Licensed Adult Day Health program serving frail elderly, younger disabled and people with Alzheimer's Disease and related dementia. Licensed for 100 a day with current average daily census of 72. RN role provides chronic disease case management, monitor medical conditions in conjunction with IDT and PCP in the community, and provides caregiver education and support. Goal of the program is to reduce ED and hospitalizations as well as prevent premature nursing home placement. EMR is Turbo TAR which is specific to CA ADHC industry so not likely anyone will know this EMR.**
