Outpatient RN Care Manager - Napa, CA - Dates: ASAP - Shift: 5x8hr Days - Years of Experience REQ: 1 year of prior RN experience REQUIRED (Clinic, Primary Care, Pre/Post, Home Health settings(s) preferred) - First-Timers Accepted: Yes - Weekend REQ: No - Certs REQ: BLS - Is on-call REQ? No - Will clinician float within scope to meet facility needs? No - Open to accommodating block schedule? No - Locals Accepted: Yes; Considered "****" within 50 mile radius, will accept locals at the same **** - Pending License Accepted: Yes (CA license REQUIRED at time of start) - RTO Restrictions: Confirm with hiring manager - Guaranteed Hours: Facility may call off traveler UP TO 1 shift per pay period (every 2 weeks) SPECIAL REQUIREMENTS - Candidate is expected to have reliable transportation and no additional scheduling restrictions. All travel pairs will be required to have their own transportation. - EPIC experience is REQUIRED - The Registered Nurse Care Manager will case manage facility's PACE participants (40 enrolled) and their medical need in clinic and in the home for their primary care needs. Support our provider NP, do initial and reassessments per PACE regulations, assist with level of care determination assessments required by the state to determine eligibility for PACE program, conduct some lab procedures, assist with medication management, participate in care planning and being part of the IDT. - Daily Patient Ratio: 1:40 - Oasis is required.
