  • Connect participants to housing resources and stability pathways
  • Coordinate referrals to healthcare, behavioral health, employment, and social services
  • Collaborate with staff, volunteers, and community partners for continuity of care
  • Maintain accurate, timely, and confidential documentation in required systems
  • Respond to crises appropriately using de-escalation and support connections

Qualifications:

  • Experience providing case management, outreach, or direct services to individuals experiencing homelessness or complex needs
  • Knowledge of trauma-informed care and ability to engage participants with empathy and professional boundaries
  • Ability to conduct assessments and develop individualized service or case plans

  • Ability to transition frequently between administrative tasks (computer/data entry) and direct participant interaction
  • Ability to bend, reach, stoop, and lift up to approximately 25 pounds on an occasional basis
  • Ability to use a computer, phone, and other standard office equipment for documentation, communication, and reporting
  • Ability to travel locally between program sites or outreach locations, as required by the role
  • Ability to visually and audibly assess situations, read documentation, and communicate clearly with participants, staff, and partners
