• Conduct comprehensive mental health assessments that take into account the medical conditions impacting our patients’ psychological well-being.
• Develop, implement, and regularly review integrated treatment plans that address both the psychiatric and medical conditions of each individual.
• Facilitate individual and group therapy sessions customized to suit each patient’s unique circumstances, employing evidence-based therapeutic modalities aimed at promoting personal growth and recovery.
• Collaborate with medical providers to ensure coordinated and effective treatment outcomes, participating actively in team meetings and care planning discussions.
• Provide crisis intervention during mental health emergencies, ensuring appropriate support is available at critical times.
• Educate patients and their families on the relationship between mental health and physical well-being, empowering them to take charge of their health journeys.
• Maintain accurate and confidential documentation in adherence to ethical and legal standards, ensuring continuous compliance with healthcare regulations.

Your work will primarily focus on individuals contending with complex mental health concerns often compounded by co-occurring medical conditions. This population requires sensitivity, understanding, and innovative treatment strategies to effectively address their multifaceted challenges. You will engage with a diverse set of clients, including veterans, ensuring that their needs are met through tailored therapeutic approaches that integrate both mental and physical health aspects.

• Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current and valid licensure as an LMHC, LCSW, LCSW-R, Licensed Psychoanalyst, Psychologist, or LMFT in the state of New York.
• Proven experience in addressing the mental health needs of individuals with co-occurring medical conditions.
• Exceptional interpersonal and communication skills to build trust and rapport with a diverse patient base.
• Ability to maintain professional boundaries and uphold ethical standards, ensuring patient confidentiality at all times.
• Familiarity with electronic health record systems and computer applications to document progress and access patient information efficiently.
• A strong team player with the ability to collaborate effectively within an interdisciplinary health care team.
