• Provide comprehensive virtual or telephonic support to patients, educating them about their treatment plans and addressing any concerns they may have.
• Serve as the primary point of contact for patients and collaborate with healthcare professionals to coordinate care effectively.
• Assess each patient's individual support needs and interaction preferences, creating personalized experiences that enable them to engage fully with their treatment pathway.
• Educate patients about program policies and procedures, ensuring clear communication and accurate documentation in clinical records.

• Master’s degree in Social Work, Counseling, or related field.
• Current LCSW license in the state of California.
