MyCHN is a nonprofit organization providing Medical, Dental, Behavioral Health, and Pharmacy services to communities in the South Houston and Gulf Coast area. The Children and Pregnant Women (CPW) Case Manager supports the care coordination program and MyCHN clinics through case management of pregnant women and children. This position involves rendering care through Texas Medicaid CPW services at MyCHN. The CPW Case Manager will help patients access needed medical, social, educational and other services, aligning with policy guidelines to ensure compliance. This position is full-time in office at following our Women’s and Children’s locations offering MFM Services and Pediatric Care.

Duties and Responsibilities:    

  • This role focuses on High Risk MFM patients through 12-months postpartum and/or on Pediatric patients with Chronic Conditions from infancy through transition to Adult Care.
  • Navigates inbound MFM referrals and initiates CPW Services
  • Navigates inbound Chronic Pediatric referrals through transition to adult care
  • Collaborates and coordinates with the providers of direct services to ensure the patient's care plan is followed.
  • Provides all aspects of case management services to pregnant women and children, from initial assessment to follow-up through program discharge.
  • Conduct non-medical drivers of health screens and follow through with referral to needed support access.
  • Participates in community events and networks with community agencies who serve pregnant women and children.
  • Actively participate in clinical huddles with the support team and various operational teams.
  • Conduct CPW coordination visits quarterly for improved health outcomes for Pediatric focus and/or the Obstetrical Program (MyPrenatal Care) visit adherence schedule. 
  • Have knowledge of, and coordinate services with, providers of health and health-related services and other active community resources.
  • Other duties assigned within scope.

Required Education and Experience:

  • High school diploma or GED 
  • Registered Nurse (RN) with an associate’s, bachelor’s, or advanced degree in nursing.
  • An RN with an associate degree in nursing must also possess two years of cumulative, paid, full-time work experience or two years of supervised, full-time educational internship/practicum experience in the past ten years with children, up to age 21, and/or pregnant women.
  • Experience must include assessing the psychosocial and health needs of and making community referrals for these populations.
  • A license must not be temporary or provisional in nature.
  • OR
  • A licensed social worker (LSW) with the appropriate licensure, including the practice of Independent Social Work.

Preferred Education and Experience:

  • HHSC Case Management Training
