MFM House Calls | Middleton, MA & Surrounding Communities
MFM Health is seeking a compassionate, experienced, and motivated Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our growing MFM House Calls team. This full-time position provides comprehensive primary care to patients in their homes throughout Middleton and the surrounding North Shore communities.
We are looking for a provider who is passionate about delivering personalized, patient-centered care beyond the traditional clinical setting. This role is ideal for someone who enjoys building meaningful relationships with patients, practicing independently while collaborating closely with a multidisciplinary team, and improving access to high-quality healthcare for individuals who benefit from receiving care at home.
Position Overview
As a House Calls Provider, you will deliver comprehensive primary care services directly in patients' homes throughout Middleton and neighboring communities. You will care for a diverse patient population, including older adults, patients with chronic illnesses, and individuals with mobility or transportation challenges.
This position offers a unique opportunity to practice relationship-based medicine by providing thorough, individualized care in the comfort of your patients' homes. You will work both independently and collaboratively with physicians, nurses, care coordinators, and specialists to ensure seamless, high-quality care.

Essential Job Duties & Responsibilities:
  • Perform comprehensive in-home patient evaluations and annual wellness visits.
  • Conduct routine follow-up appointments and chronic disease management in patients' homes.
  • Assess, diagnose, and treat acute and chronic medical conditions.
  • Develop individualized treatment plans in collaboration with supervising physicians and the interdisciplinary care team.
  • Order, interpret, and review laboratory studies, diagnostic imaging, and other clinical tests.
  • Prescribe medications and therapeutic interventions in accordance with state and federal regulations.
  • Review emergency department visits, hospital admissions, and specialist consultations, providing timely follow-up care.
  • Perform medication reconciliation and optimize treatment plans to improve patient outcomes.
  • Coordinate care with specialists, home health agencies, rehabilitation providers, hospice organizations, and community resources.
  • Educate patients and caregivers regarding disease management, medications, nutrition, fall prevention, and healthy lifestyle practices.
  • Maintain accurate, timely electronic medical records documenting all patient encounters.
  • Identify social determinants of health impacting patient care and collaborate with the care team to address barriers.
  • Participate in quality improvement initiatives and practice development efforts.
  • Travel throughout Middleton and surrounding communities to provide scheduled patient visits. MFM Health vehicle is provided for home visits.
  • Perform other related duties as assigned.
Education & Qualifications
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • Current Massachusetts Nurse Practitioner or Physician Assistant license.
  • National certification as a Family Nurse Practitioner or Physician Assistant.
  • Minimum of 2 years of clinical experience as a Nurse Practitioner or Physician Assistant preferred.
  • Valid Federal DEA registration and Massachusetts Controlled Substance Registration (MCSR).
  • Current BLS certification.
  • Valid driver's license, reliable transportation, and willingness to travel throughout the service area.
  • Excellent clinical assessment and diagnostic skills.
  • Strong communication, organization, and time management abilities.
  • Ability to work independently while collaborating effectively within a multidisciplinary healthcare team.
  • Experience in primary care, geriatrics, home-based primary care, internal medicine, family medicine, or value-based care is strongly preferred.
  • Commitment to compassionate, patient-centered care and continuous professional development.
Compensation
  • $115K - $150K; commensurate with experience
