Job Summary:
Location: Methodist Jennie Edmundson Hospital
Address: 933 E Pierce St, Council Bluffs, IA
Work Schedule: Full-Time benefit eligible 24 hrs./wk., 8am-noon, M-F, semi flexible hours with additional evening and weekends hours, as needed.
Supports the growth, sustainability and measurable impact of the Caring for Our Communities initiative. Provides specialized support in data tracking, grant writing, grant reporting, program documentation and outcome reporting to help demonstrate the value of community health work across Methodist Jennie Edmundson Hospital, the Jennie Edmundson Foundation and community partners.
Responsibilities:
Essential Functions
Data Collection, Tracking, and Reporting (50%)
  • Collect, organize, and maintain program data related to referrals, services provided, flexible funding supports, Community Health Coach activity, partner engagement, and program outcomes.
  • Support data tracking through systems such as Unite Us, internal spreadsheets, hospital reports, partner updates, and other approved tools.
  • Prepare regular data summaries for Community Health leadership, hospital leadership, board committees, donors, and external funders.
  • Compile data for ROI analysis, grant reporting, community benefit reporting, program evaluation, and strategic planning.
  • Review data for accuracy, consistency, and completeness, and help identify gaps, trends, and follow-up needs.

Grant Writing and Funding Support (30%)
  • Draft, edit, and prepare grant proposals, letters of inquiry, funding requests, and supporting documents for Caring for Our Communities and related community health programs.
  • Gather program information, budgets, partner details, outcome data, community impact examples, and other documentation needed for grant submissions and reports.
  • Maintain a calendar of grant deadlines, reporting requirements, funding opportunities, and follow-up needs.
  • Research potential funding opportunities aligned with social determinants of health, health equity, access to care, behavioral health, transportation, housing stability, food security, and support for vulnerable populations.
  • Prepare funder updates, stewardship reports, board reports, and donor-facing impact summaries.

Program Documentation and Communication (10%)
  • Develop and maintain clear written descriptions of Caring for Our Communities programs, Community Health Coach roles, partner collaborations, referral pathways, and funding priorities.
  • Work with Community Health Coaches and program leaders to identify patient or community impact stories while maintaining confidentiality and appropriate privacy standards.
  • Translate program results into clear narratives for grants, reports, presentations, donor communications, and community updates.
  • Support the development of one-page summaries, talking points, program overviews, dashboards, and presentation materials.

Community Health Program Support (10%)
  • Develop and maintain clear written descriptions of Caring for Our Communities programs, Community Health Coach roles, partner collaborations, referral pathways, and funding priorities.

  • Ensure written materials are accurate, professional, and aligned with Methodist Jennie Edmundson Hospital and Jennie Edmundson Foundation messaging.

Job Requirements
Education:
  • Bachelor's degree in public health, nonprofit management, health administration, communications, social work, human services, business or a related field preferred. Equivalent experience may be considered.

Experience:
  • Two or more years' experience in grant writing, program coordination, community health, nonprofit work, data reporting, public health, healthcare administration or related work required.
  • Experience collecting, organizing and summarizing data required.
  • Experience with referral platforms, CRM systems, electronic health records, Unite Us or other data/reporting systems required.

License/Certification:
  • N/A

Skills/Knowledge and Abilities:
  • Strong writing, editing, proofreading, organizational and project management skills.
  • Ability to manage multiple deadlines and projects with attention to detail.
  • Proficiency with Microsoft Office, including Word, Excel, Outlook and PowerPoint.
  • Knowledge of health equity, vulnerable populations, homelessness, behavioral health, transportation barriers, food insecurity, housing instability or access-to-care challenges.
  • Ability to work independently while collaborating with multiple internal and external stakeholders.
  • Excellent written and verbal communication skills.
