In this highly technical, fast-paced, and challenging position, you'll collaborate with multidisciplinary team members to provide the very best care for our patients. The Community Health Worker provides proactive individualized coaching and resource linking to a group of patients via the telephone and/or home visits with an end goal of affecting patient healthy living and behavioral change.
Your Job Requirements:
• High school diploma or equivalent, Bachelor degree preferred
• 2 years health coaching, wellness, health promotion, health outreach experience. Health education or weight management background preferred
• Community Health Worker Certificate or ability to obtain within one year from date of hire
