Registered Nurse (RN) – Home Health | Per Diem

Bella Terra Home Health and Hospice is dedicated to providing comfort, compassion, and exceptional care to patients and families. Our mission is to enhance quality of life through personalized hospice services delivered by a team committed to excellence.

We are growing and seeking an experienced, compassionate Home Health Registered Nurse (RN) to join our team on a per diem basis in the Inland Empire area!

Key Responsibilities

  • Develop and implement individualized plans of care in partnership with patients, caregivers, and interdisciplinary teams.
  • Provide preventative and rehabilitative nursing procedures as ordered by the physician.
  • Deliver patient, caregiver, and family education and counseling.
  • Prepare and maintain accurate clinical notes.
  • Communicate effectively with physicians and healthcare team members regarding patient needs and changes in condition.
  • Identify discharge planning needs and implement prior to patient discharge.
  • Supervise Licensed Practical/Vocational Nurses (LPN/LVN) and Home Health Aides (HHA/CNA) as directed.
  • Participate in on-call duties, quality improvement programs, and in-service training.

Qualifications

  • Active California RN License
  • Minimum 2 years of nursing experience (home health preferred)
  • Current CPR certification
  • Reliable transportation and valid driver’s license with insurance
  • Strong observation, problem-solving, and communication skills
  • Ability to work independently and prioritize workload
  • Physical ability to lift, transfer patients, and perform nursing duties
