We are seeking a compassionate and skilled Memory Care Coordinator to join our team at Grand Villa Assisted Living in Winston-Salem, North Carolina. As a leading provider of assisted living services, we are dedicated to delivering exceptional care and support to our residents. Our Memory Care Coordinator will play a critical role in ensuring the well‑being and quality of life of our residents living with memory impairments. MUST HAVE A NORTH CAROLINA MEDICATION TECHNICIAN CERTIFICATION.

About the Role:

As a Memory Care Coordinator, you will be responsible for coordinating and delivering personalized memory care services to our residents. This includes:

  • Developing and implementing individualized care plans in collaboration with the care team
  • Providing emotional support and guidance to residents and their families
  • Facilitating resident activities and socialization programs
  • Collaborating with caregivers and other healthcare professionals to ensure seamless care transitions
  • Maintaining accurate and up-to-date records of resident care and services
Responsibilities:
