We are seeking Memory Care Coordinator Assistant to join our team in our Coraopolis, PAlocation.
The duties of the Memory Care Coordinator Assistant involve providing escort service, direct care and program assistance to Individuals with Intellectual and Developmental Disabilities (IDD) and Alzheimers /Dementia (AD/D). This position requires the candidate to work with individuals with IDD in a Memory Care Home setting. All work is performed under the direction of the Memory Care Coordinator and the Program Director in the areas of assignment.
Responsibilities Include:
  • Assists the residents' activities of daily living; such as, toileting, feeding and other personal hygiene needs.
  • Escorts residents to and from scheduled program classes.
  • Implements individual goal plans, alternate and /or back-up activities, and documents residents' attendance and progress in Day Program
  • Provides guidance in social, recreational, educational and therapeutic activities.
  • Assists in organization and conduction of special events and off-campus trips.
  • Offers direction to Residential Services staff working in Program areas/Memory Care Home.
