Job description


The Medication Management Clinical (MMC) Pharmacist shall be responsible for providing pharmaceutical care services in the ambulatory care setting utilizing approved protocols and following established policies. Provides customer focused clinical pharmaceutical care which includes, but is not restricted to primary care, anticoagulation services, and chronic condition management. Works under the direction of and report to the Program Director of Medication Management. Works in collaboration with the multidisciplinary team, identify clinical interventions, and provide medication safety across the continuum. Serve as a medication information specialist by disseminating pertinent information to physicians, patients, nursing, and other clinical staff deemed appropriate. Participates in patient and provider educational programs to positively impact patient compliance and provider prescribing practices. Provides pharmaceutical care through continual monitoring of patients' drug therapies and accepting responsibility for outcomes. Responsible for maintaining the security of any clinic medication supplies. Effectively utilizes pharmacy technology driven systems to improve patient safety and streamline workflow. Responsible for quality reports, medication utilization evaluations, performance improvement activities and other projects/duties as assigned adhering to deadlines set forth. All efforts shall be coordinated with and have the approval of the designated physician(s) and/or Program Director of Medication Management.

Required Skills
  • Possess advanced pharmacokinetics and clinical knowledge of the disease state(s) of the patient population being treated and have the ability to apply this knowledge in an Ambulatory Care setting.
  • Critical thinker and effective educator.
  • Possess knowledge of medication related polices/protocols, clinical trials, evidence of continuous professional training in related field of work, excellent computer, verbal/written communications, and ability to prioritize.
  • With consideration to age, utilizes the approved process to resolve biophysical, psychological, educational, and environmental needs of patient/significant other when administering care.

Required Experience
Required:
  • PharmD degree from an accredited school of pharmacy recognized by the California State Board of Pharmacy.
  • Must be a California Registered Pharmacist.
  • Must have a current BCLS Healthcare Provider CPR Card from the American Heart Association.
  • Completion of an ambulatory care residency and/or advanced certification in a pharmacy specialty area.
  • A minimum of three (3) years working experience as a California Registered Pharmacist in a hospital or ambulatory care setting with clinical background in a specialty area.

Preferred:
  • Experience with design of pharmacy systems, equipment specific to medication being administered and processes preferred.
  • Bilingual preferred
