Job Description

The home health Registered Nurse is responsible for providing direct home health nursing care for a team of clients in accordance withnursing practice, Medicare regulations, agency standards and policies.

Specific responsibilities include:

  • Completes and documents skilled nursing assessments for patients following physician orders.
  • Collaborates with physicians, other health care professionals, clients and families in developing a comprehensive coordinated plan of care.
  • Develops an individualized plan of care which includes objectives goals for client improvement in health and functional status. Evaluates plan of care on a continuous basis, updates physician(s) and other therapies of changes in client status and need for revision of plan of care.
  • Delivers skilled nursing services efficiently and effectively to meet client care needs.
  • Ensures that physician orders are communicated to the agency's staff with accuracy, thoroughness and understanding.
  • Initiates case conferences to discuss multidisciplinary team responsibilities, client progress, plans for continued care, new goals, etc.
  • Plans for discharge from the agency with the client and caregivers. Makes referrals for alternative level of care if necessary.
  • Follows schedule frequency as ordered by physician.
  • Proactively participates in care conferences with other health team members as needed.
  • Maintains privacy and confidentiality of client, client's records and client's living environment. Understands and follows HIPAA guidelines.
  • Promotes personal safety and a safe environment for clients and co-workers.
  • Other duties as assigned.
Qualifications

The following are the minimum required qualifications for the home health RN position:

  • Graduate from an accredited school of nursing as a Registered Nurse (RN).
  • Maintains a current, unencumbered Registered Nurse license in the State of Minnesota.
  • One (1) year experience as a Registered Nurse in home health.
  • Knowledge of Medicare home health regulations and OASIS data-sets.
  • Demonstrated leadership and judgement with strong knowledge in nursing practices.
  • Must understand and communicate with specialized medical terminology and explain procedures to clients.
  • Valid driver's license, current auto insurance and reliable vehicle.
  • Ability to communicate effectively, including written and verbal skills.
  • Ability to work work well both individually and as part of a team, through respect, collaboration and dedication; successfully interact with a diverse culture base.
  • Ability to prioritize and organize work effectively and efficiently.
  • Demonstrated positive attitude, high integrity and ethical awareness.
  • Proven to be dependable and responsible.
  • Compassionate and caring attitude.
