Job description

LPN - Home Health - Full Time
80 hrs. Biweekly - Mon-Fri 8am - 430pm

About Meritus Home Health:

For over 25 years, our team of medical professionals has provided the full spectrum of care to our community in the Tri-State region, ranging from skilled nursing to assistance with the activities of daily living. We provide patient care services every day of the year, have cared for over 18,000 patients and currently have more than sixty employees.  We are accredited by the Joint Commission and certified by both Medicare and Medicaid. 

Under the general direction of the clinical manager/manager of assigned department and the direct supervision of a registered nurse, performs in a team relationship providing care for patients using the nursing process, judgment, and evidenced based practice to assess and treat responses to actual or potential health problems. Maintains clinical competency and participates in continuing education related to health care issues and as outlined by appropriate regulatory and accrediting agencies. Adheres to ethical principles of practice as defined by the Nurse Practice Act and the ANA code of ethics for nurses.  Demonstrates ongoing competency in practice. Reports breaches in clinical or ethical standards to supervisor.

Education – Graduate from an approved nursing program as a Licensed Practical Nurse or Licensed Vocational Nurse.

Experience – Current BLS (CPR) in compliance with policy. Three years of experience in an acute care or rehab setting is required. Home Health experience is preferred.  

Licensure/Certification – Current license to practice in the State of Maryland. 

For use of Personal Vehicles: Access to a vehicle and current driver’s license from any state is required. Acknowledgement with signature to “Attestation A” of Meritus Vehicle Management Use and Control policy, which includes guidelines for personal insurance coverage requirements.

Knowledge/Skills/Abilities – Ability to apply the standards of professional nursing practice with compassion and empathy. Basic computer skills required. Must posess high level of organizational and communication skills.
