The Licensed Professional Counselor will:

• Assist primary care providers by identifying, treating, and managing mental health and psychosocial issues.
• Conduct comprehensive patient intakes that encompass diagnostic and functional evaluations, and make informed treatment recommendations to primary care providers.
• Deliver brief, focused interventions for patients requiring mental health services, tailoring approaches to meet specific patient needs.
• Facilitate follow-up communications to evaluate patient progress and adapt treatment plans as necessary.
• Develop and implement relapse prevention plans in collaboration with patients, aimed at maintaining stability during recovery.
• Monitor the delivery of behavioral health services and coordinate care with other treatment providers, ensuring comprehensive support for patients.
• Document patient progress diligently in treatment records, maintaining clear communication with primary care providers.
• Offer guidance to patients on self-management strategies to empower them in their treatment and recovery journey.
• Advise clinic management regarding behavioral services, program evaluation, and continuous improvement initiatives.
• Uphold the Code of Ethical Conduct for Clinical Professionals and fulfill other duties as assigned.

As an LPC at Oasis Psychotherapy Services, you will serve a diverse clientele that includes children, adolescents, and adults dealing with various mental health issues. • A current LPC (Licensed Professional Counselor) license in the state of Colorado.

• Effective communication and interpersonal skills, with an ability to build rapport with patients and collaborate within an interdisciplinary team.

While not required, the following experiences are preferred:
