Support Patients Beyond Their Medical Needs.

Vision Healthcare is seeking a compassionate Medical Social Worker (MSW) to help patients and families navigate the emotional, social, financial, and community resources needed to achieve the best possible health outcomes.

As part of our interdisciplinary home health team, you'll advocate for patients, connect families with community resources, and provide counseling and support during some of life's most challenging moments.

Responsibilities

  • Complete psychosocial assessments.
