Job description

JOB DESCRIPTION

FUNCTION

The primary purpose is to provide each assigned resident with routine daily personal care and services in accordance with medical directives, hospice care plan and interdisciplinary group meetings. Demonstrate and communicate the mission, ethics and goals of the facility as the face of Hospice. Professionally represent the organization with credibility, compassion and dignity and in the spirit of excellence and trust.

SUPERVISION RECEIVED

Reports to the lead Certified Nursing Assistant. The lead Certified Nursing Assistant works under the supervision of the assigned nurse and/or charge nurse. All positions report directly to the Director of Inpatient Services.

EXAMPLES OF WORK PERFORMED

Job Summary:

Direct Care:

·        Provides personal care and assistance to assigned patients per the worksheet and reports any changes to the registered nurse or LPN.

·        Reports changes in the patient’s condition to the assigned nurse and/or Lead Certified Nursing Assistant.


·        Ensure continuity of care until relief arrives. Ensure a smooth and safe hand off via shift to shift report to all related personnel (i.e. Lead Certified Nursing Assistant, oncoming certified nursing assistant

Food Service Functions (as needed):

·        Prepares residents for meals. Serves food trays and assist with feeding as needed.

Housekeeping Functions:

·        Performs routine housekeeping duties as assigned using housekeeping procedures.

HIPAA:

·        Ensures the protection of patient and employee information by following HIPAA-HITECH guidelines.

·        Maintain the integrity of all forms of patient information including but not limited to electronic personal health information.

·        Assures that there are no attempts to access more than the minimum patient information necessary to complete his/her job.

Adheres to all compliance and policy/procedure requirements of Hospice of the Upstate related but not limited to:

Certification renewal, regular attendance and punctuality is an essential function of all jobs at Hospice of the Upstate, safety protocols, dress code policy, HIPAA, infection control and employee training (computer based, webinars, departmental educational classes, 75% required department staff meetings, and 75% required Hospice of the Upstate staff meetings. To perform required continuing education hours of one (1) hour per month or 12 hours per year.

Other:

·        Works holidays and weekends as needed. Must be willing to work shifts other than those hired for as required to provide safe, appropriate patient care.

·        Assumes other duties as necessary.

DESIRED MINIMUM QUALIFICATIONS

Education and Experience:

·        Must be a Certified Nursing Assistant.

Necessary Knowledge, Skills and Abilities:

·        Must be empathetic, responsible, able to use time effectively, and to accept supervision.

·        Able to verbally communicate with the residents and residents’ families compassionately.

·        Able to maintain pleasant working relationships.

·        Able to perform multiple tasks simultaneously.

·        Able to work successfully with varied groups of people.

·        Be flexible, organized and work professionally under stress.

TOOLS AND EQUIPMENT USED

Standard Certified Nursing Assistant equipment.

PHYSICAL DEMANDS

May be required to climb steps, stoop, bend or lift up to 50 pounds. Must be able to stand for long periods of time.
