Job description

Palm Medical Centers is a rapid growing company currently seeking an experienced RN Clinical Coordinator with leadership experience to join our team.
The RN Clinical Coordinator Manager is responsible for supporting and overseeing the Case Management (CM) team within the Central Florida (CFL) Market. This role ensures effective coordination and continuity of care for patients across hospital and skilled nursing facility (SNF) settings, from admission through discharge.
The position reports directly to the Utilization Management Department.

Key Responsibilities:
  • Supervise assigned Case Managers, including monitoring monthly performance metrics and providing ongoing coaching and development
  • Coordinate team coverage to ensure continuity of operations
  • Conduct onboarding and orientation for new staff
  • Maintain daily tracking of all hospital and nursing home admissions, readmissions, and discharges for assigned centers
  • Lead education, training initiatives, and staff development programs
  • Monitor case activity to identify trends, risks, and opportunities for improvement
  • Communicate effectively with Regional leadership, Administrators, and providers regarding patient care needs and operational priorities
  • Identify and escalate potential catastrophic and stop-loss cases
  • Recognize patients with potential subrogation or cost allocation opportunities
  • Coordinate patient transfers as directed by the Medical Director or Primary Care Provider (PCP)
  • Support PCPs with appropriate patient placement in Skilled Nursing Facilities (SNFs)
  • Participate in Case Management huddles and monthly meetings
Operational & Strategic Functions:
  • Enhance staff competency by delivering education, facilitating learning opportunities, and ensuring application of knowledge to improve patient outcomes
  • Improve care delivery and outcomes by evaluating, redesigning, and implementing processes, policies, and procedures
  • Contribute to financial performance by analyzing results, monitoring variances, identifying trends, and recommending actionable solutions
Requirements:
  • Proven experience in case management, including roles such as RN Case Manager or healthcare care manager
  • Demonstrated experience managing or supervising staff
  • Strong knowledge of case management principles, utilization management, and reimbursement processes
  • Understanding of the psychological and behavioral aspects of patient careโ€‹โ€‹โ€‹โ€‹โ€‹โ€‹โ€‹
  • Excellent communication, organizational, and time management skills
  • โ€‹โ€‹โ€‹โ€‹โ€‹โ€‹โ€‹Ability to multitask, problem-solve, and work effectively in a team-oriented environment
  • Compassionate, patient-centered approach to care
  • Familiarity with CPT coding and ICD-10 terminology
  • Knowledge of InterQual or Milliman Care Guidelines preferred
  • Bilingual skills preferred
  • โ€‹โ€‹โ€‹โ€‹โ€‹โ€‹โ€‹Active Florida Registered Nurse (RN) license

Minimum Qualifications:ย Active FL RN license, minimum 3 years of RN Clinicalย  experience.
