The program acts as a liaison between CHR, CSSD/JBCSSD, clients, families, and community partners to support timely access to services, reduce barriers to treatment, and promote successful engagement in care. The program aims to assists youth and families with identified needs related to behavioral health, substance use, medical care, housing, education, employment, legal matters, transportation, benefits, family support, and other activities of daily living, while completing administrative, programmatic, triage, and documentation responsibilities in accordance with program and contract expectations. Acts as primary point person for program and provides back up coverage as needed by program and agency. Ensures proper maintenance of vehicles/if applicable. Oversees completion of chart audits. Ensures completion of insurance authorization for services rendered as necessary.
