Primary Care Physician (PCP) Responsibilities
  • Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients.
  • Coaching for health: The PCP acts as a health coach, helping patients set short- and long-term health goals, partnering with them and following up to improve health outcomes.
  • Simplifying for action: The PCP simplifies and prioritizes care to make behavior change more actionable and leads care teams toward performance goals.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES
  • The PCP independently provides care for older adult patients with acute and chronic illnesses, takes full accountability for patient care and outcomes, and seeks specialist consultation when needed while remaining actively responsible.
  • The PCP engages with hospitalists and care teams for hospitalized patients and coordinates transitions of care, documenting all details.
  • The PCP leads a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for office-visiting patients, and collaborates with transitional care teams for homebound or facility-based patients.
  • The PCP has an active management role in the center, covers for other providers as needed, and participates in recruiting patients and additional providers.

PAY RANGE

$221,141 - $315,915. Salary depends on experience, education, location, and other factors. Bonus or commission may also be available.
