This position is based onsite in a hospital setting in Louisville, Colorado, known for its supportive healthcare community and breathtaking surroundings. Here, you will work closely with a diverse patient population, primarily older adults seeking rehabilitation and wellness services. The selected candidate will be responsible for a variety of daily functions, which include but are not limited to the following:

• Delivering physical therapy and wellness services to older adult patients under the guidance of a licensed Physical Therapist, utilizing evidence-based practices to maximize treatment effectiveness.
• Conducting regular wellness checkpoint visits with patients to monitor their progress and report findings to the supervising PT.
• Facilitating telehealth visits by preparing patients, providing educational materials, and liaising with caregivers and healthcare providers.
• Implementing care plans that aim to optimize each patient’s functional well-being while ensuring compliance with ethical standards and internal policies.
• Contributing to program development initiatives, participating in quality improvement projects, and engaging in problem-solving to enhance service delivery.

Your primary focus will be on older adults who require physical therapy for conditions related to mobility, strength, balance, and overall wellness. You will assess individual needs and implement tailored care plans that address specific physical challenges faced by this demographic. You will also engage families and caregivers, taking an active role in their understanding of treatment plans, which is crucial for successful rehabilitation.

• A completed and accredited Physical Therapy Assistant program, and new graduates are encouraged to apply.
• Must hold current state licensure as a Physical Therapist Assistant in Colorado.
• Demonstrated excellent verbal and written communication skills essential for patient interaction and collaboration with healthcare teams.
• A results-oriented mindset, with critical thinking skills to assess patient needs and modify treatments effectively.

• A minimum of two years of experience in a physical therapy setting, particularly with older adults or geriatric populations.
• Proficiency in the use of electronic health records (EHR) and telehealth platforms will be advantageous.
