• Deliver direct physical therapy interventions as per the direction of the supervising physical therapist, adhering strictly to established care plans tailored to each patient's unique requirements.
• Administer therapeutic treatments aimed at alleviating pain, restoring function, and promoting maximum performance in patients with varying physical limitations.
• Lead and assist patients through both active and passive therapeutic exercises, while facilitating muscle reeducation, gait training, and assistance with daily functional activities.
• Employ various physical agents (such as heat, cold, electrical stimulation, ultrasound, and massage) to enhance treatment effectiveness and support patient recovery.
• Carefully observe patient reactions to interventions, documenting significant changes and communicating findings to the supervising therapist and relevant healthcare staff via established protocols.
• Educate patients on the appropriate usage of mobility devices such as wheelchairs, braces, or orthotic and prosthetic devices to promote independence in their rehabilitation process.
• Maintain meticulous clinical documentation including therapy notes and conference summaries, ensuring timely integration into the patient’s clinical record, thereby supporting continuity of care.
• Actively participate in interdisciplinary educational programs and quality improvement activities, contributing insights that enhance service delivery and patient care.
• Perform any additional responsibilities or projects as assigned by the supervising therapist or senior management team.

In this role, you will primarily work with a diverse range of patients, including those recovering from surgical procedures, managing chronic conditions, or seeking rehabilitation after injuries. Our patient-centric approach emphasizes individualized treatment plans, and you will play a crucial role in enabling patients to achieve their maximum functional potential.

• A graduate of an accredited physical therapy assistant program with an Associate's degree in science or applied science, recognized by the American Physical Therapy Association.
• Possession of a current Physical Therapist Assistant license valid in New York state, along with CPR certification.

• Must possess a valid driver's license and a reliable vehicle, insured as per state requirements for travel to patient homes if required.

• A minimum of two years of clinical experience in physical therapy, preferably within a hospital or comparable healthcare setting, is strongly preferred.
