Key Responsibilities
  • Conduct thorough assessments and evaluations to identify the unique mental health needs of each patient, ensuring personalized care plans.
  • Facilitate ongoing communication with multidisciplinary teams, including supervisors, direct care staff, and family members, to track and report on patient progress.
  • Develop a proactive schedule that incorporates regular appointments, home visits, and progress monitoring activities.
  • Coordinate home visits, chart reviews, and staff interviews to ensure a comprehensive understanding of patient situations and adherence to program plans.
  • Actively participate in Interdisciplinary Team Meetings and provide valuable input during discussions that affect patient care.
  • Maintain accurate and timely documentation, including monthly and quarterly progress notes, to adequately reflect the patient journey.
  • Design and implement training for staff on program plans and documentation procedures to ensure consistent implementation of best practices.
  • Provide follow-up support through interviews and ongoing assessments to ensure sustained improvement in patient outcomes.
  • Serve as a liaison between patients, their families, and the mental health community to advocate for and support individual recovery goals.
  • Assist in the creation of training materials to enhance program efficacy and staff competence.

Qualifications
