Key Responsibilities
  • Conduct ongoing and comprehensive assessments to identify the unique needs of each patient, ensuring tailored mental health support.
  • Facilitate clear and open communication among supervisors, direct care staff, family members, and other stakeholders regarding each individualโ€™s progress and treatment goals.
  • Prepare and provide a proactive schedule for supervisors, detailing forthcoming appointments, home visits, and observation timelines to ensure effective monitoring of each clientโ€™s progress.
  • Coordinate essential home visits that include conducting in-home observations, chart reviews, staff interviews, and thorough data analysis.
  • Ensure that all program plans are effectively implemented and adjusted as necessary to meet client needs.
  • Participate actively in Interdisciplinary Team Meetings and other critical discussions to enhance collaborative treatment approaches.
  • Complete and submit thorough monthly and quarterly progress notes, documenting advancements and adjustments in care.
  • Organize and lead training sessions aimed at ensuring staff and team members are proficient in implementing the program plans and documenting care.
  • Conduct follow-up interviews, home visits, and client observations as necessary to provide continued support and assessment of ongoing needs.
  • Maintain accurate and organized records, ensuring that vital information is readily accessible to authorized personnel.
  • Act as a liaison with the broader mental health community and public to foster support and development of mental health initiatives.
  • Contribute to the creation and improvement of training materials and program processes for enhanced client care.

This role provides an opportunity to work with a diverse range of clients, including both adults and youth. Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

  • Specialization in areas such as family therapy, couples therapy, substance misuse treatment, or trauma counseling is highly preferred.
