As a MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident's unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect by developing, monitoring, auditing, and modifying each resident's care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. To be eligible for consideration applicants should have:
As a minimum, an unencumbered State of Illinois License, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as a nurse in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
